WAIVER / RELEASE OF LIABILITY FOR PARTICIPANTS READ BEFORE SIGNING
In consideration of (myself/my child) being allowed to participate in any way in Tulare Adult Soccer League and City of Tulare related events and activities, my signature acknowledges, appreciates, and agrees that: The risk of injury to (myself/my child) from the risk involved with these programs is significant. Including the potential for permanent disability and death. While particular rules and regulations, equipment, and personal discipline may reduce the risk, the risk of serious injury does exist.
1. FOR MYSELF, SPOUSE, AND CHILD I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown. EVEN IF ARRISING FROM THE NEGLEGINCE OF THE RELEASES OR OTHERS, and assume full responsibility for (myself/ my child's) participation.
2. I willingly agree to comply with the programs stated and customary terms and conditions for participation. If I observe any unusual significant concern in (myself/ my child's) participation and/or in the program itself, I will remove (myself/ my child) from participation and make the nearest official aware of the situation.
3. I myself, my spouse, my child and on behalf of my/our heirs, assigns personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS Tulare Adult Soccer League or City of Tulare, its directors, officers, officials, agents, employees, volunteers, other participants, sponsoring agencies, sponsors, advertisers, and if applicable owners and lessors of premises used to conduct events, (Releasees), WITH RESPECT TO ANY AND ALL INJURY, DISABILIT, DEATH, or loss or damage to person or property incident to (myself/ my child) involvement or participation in these programs. WETHER ARRISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law.
4. I myself, my spouse, my child and on behalf of my/our heirs, assigns personal representatives and next of kin, HERABY INDEMNIFY AND HOLD HARMLESS all above releasees from any and all liabilities incident to my involvement or participation in these programs. EVEN IF ARRISING FROM THEIR NEGLEGENCE, to the fullest extent permitted by law.
5. I myself / parent / guardian, assert that I have explained to my child/ward the risk of the activity and his/her responsibilities for adhering to the rules and regulations, and that my child/ward understands this agreement.
I FOR MYSELF, MY SPOUSE, MY CHILD/WARD, HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT WE HAVEN GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCMENT.
I understand the seriousness of the risks involved in participating in the program, my personal responsibilities for adhering to rules and regulations and accept them as a participant.
